Reporting Quarters

Quarter 1: April 1, 2026 – June 30, 2026

Quarter 2: July 1, 2026 – September 30, 2026

Quarter 3: October 1, 2026 – December 31, 2026

Quarter 4: January 1, 2027 – March 31, 2027


Deadlines:

  • Quarterly ORS submissions are due on the last day of each quarter by 9:00 am.
  • Quarterly narrative reports are due no later than the second Friday after the quarter has closed. Please email directly to the Network Coordinator lisa@rogozinsky.org
  • 6-month Expenditures (April 1, 2026 – September 30, 2026) in the form of a Schedule B are due October 15, 2026 (this is not required for agencies that provide invoices for payment).
  • Un-audited Year-end Schedule B are due April 30, 2027 (this is not required for agencies that provide invoices for payment).

Audit Requirements:

  • For contracts over $500,000, the contracted service provider must provide either an
    audited Schedule B or an audited Financial Statement with the accompanying Auditor’s
    Report, or equivalent audited financial documentation supporting the invoices
    submitted to EFAN. These must be provided within 90 days of the reporting date(s)
    specified in the Letter of Agreement, or 90 days after contract termination.
  • For contracts more than $250,000 but less than $500,000, the contracted service
    provider must provide either an un-audited Schedule B certified by two authorized
    signatories with a Review Engagement Report, or a summary of invoiced program
    expenditures verified by the agency’s financial officer and supported by a Review
    Engagement Report, within 90 days of the reporting date(s) or contract termination.
  • For contracts less than $250,000, the contracted service provider must provide either
    an unaudited Schedule B certified by two authorized signatories or a certified summary
    of invoiced program expenditures within 90 days of the reporting date(s) or contract
    termination.

Narrative Reporting

Narrative reporting provides qualitative insights that numbers alone cannot communicate. It aims to offer a more comprehensive understanding of program activities, including the reasons for expected and unexpected results.

Below are key qualitative areas that should be reported on per program.

  • What activities were actioned?
  • Where did these activities occur?
  • Who did these activities serve?
  • Did you collaborate or partner with any other groups or organizations? Referrals to other supports/organizations?

What changed or improved for:

  • Clients?
  • Clients’ support systems?
  • The community?
  • What delayed or limited your impact?
  • What will you do differently in the future?
  • What are next steps?

How do you include practices that are:

  • Accessible?
  • Culturally appropriate and safe?
  • Inclusive?
  • Trauma-informed?

Reviewing and Submitting

Before submitting your narrative report, complete a check using the sections below. Consider breaking up your review into segments to cover the entire document without fatigue or ask a colleague to review. The “read aloud” function in Word can also be helpful.

Clarity and Understanding
  • Alignment with Objectives: Does the report address the objectives outlined in the Schedule A of the Agency Service Letter of Agreement? 
  • Use Plain Language: Could a family understand your report?
  • Highlight Purpose: Is the information’s purpose clearly defined? Why is the information important and what should the reader identify or understand after reading?
Accuracy and Formatting
  • Voice and Tone: Are living experiences of clients, families, and community members described, with consideration for their context and emotions?
  • Perspective: Does the description acknowledge the impact to the individual or group, as well as the importance of programs and services provided?
  • Formatting: Is the font, font spacing, bullet styles, figures and tables consistent throughout? Reports should be approximately 500 words.
